A safety assessment of the home can help reduce the risks in at home and make safety suggestions to assist in creating a safe home environment. We can develop a plan of care as well as provide professional and community resources.

A Care Manager can assist families with developing a plan of care for your loved one, the coordination of services, assist a family through a crisis as well as be your advocate when you can’t be here. For families that have a loved one living at a distance, a Care Manager can provide wellness checks and follow up on services as well as appointments.
